Default Test Creation Survey

Hi, I'm conducting a survey to understand how teachers create tests. The survey results will help find the advantages, disadvantages, and possible improvements to the different creation processes.

All answers to the survey will be treated anonymously, with any board information removed.

1) Are you more likely to use a prefabricated test or create one of your own?
2) How do you gather questions before, or during, test creation?
3) How do you write your test? What tools (Microsoft Word, etc.) do you use?
4) What different formats, overall and question types, do you commonly use for your tests?
5) What do you find easiest about your test creation process?
6) What do you find most difficult about your test creation process?
7) What takes the most time to accomplish in your test creation process?
8) What takes the least amount of time in your test creation process?
9) What would you like to change in your test creation process?
10) What would you keep consistent in your test creation process?
11) What subject(s) do you teach?

Thanks for your time, I appreciate your help.
