interactive database

Please help! this year I will be teaching special education work coop. and information technology. Anyone who has a template for an interactive behavior management plan- for tracking performance and participation (an interactive database) Also, any suggestions regarding software, internet sites or other resources that would be helpful. These particular students are Jrs and Srs, low functioning, very few computer skills. My school uses windows. thanks!
