Can you help with Special Needs Assignment?

Hello!
I'm currently doing a Post Grad Diploma in Special Education and was wondering if anyone could help me with an assignment we have been set.

We have to design a Transition Portfolio that will provide next year's teacher with information about a child from this year's class. This child will have a special need ( in my case I have chosen Mosaic Down Syndrome) and we have to provide the next teacher with relevant information.

I was wondering what sort of information you would want - if you were the incoming teacher, and if you were the 'old' teacher?

If you need information on "transition planning"for a special needs student", I would suggest that you search on the internet under "transition" with google you could go "transition + elementary to middle school" or "middle school to high school". This should give you an idea as to what information you can provide for the next level.
